Provides support for seniors (65+) living in Invermere to access non-clinical, community-based services.
The Social Prescribing Program supports seniors aged 65 plus to access non-clinical, community-based services to prevent or delay frailty by fostering resilience and social support using a comprehensive, strength-based approach. Social Prescribing is an approach to offering non-medical support that enables older adults to identify,
set and achieve personal health and wellness goals. A Community Connector helps to find appropriate local programs and services to enhance the quality of life for seniors.
This program is intended for individuals residing in the Columbia Valley area 65+ who is either experiencing or is at risk of:

- Frailty
- Social Isolation and/or Loneliness
- Barriers to Accessing Community-Based Resources
- Frequent Trips to Emergency Department or Primary Care
- Experiencing Life-Limiting Illness (Congestive Heart Failure, Renal Failure, COPD, Diabetes, etc.), End-of- Life, or Palliative Care
