Vascular Improvement Program - Cardiac Rehabilitation

Provides cardiac rehabilitation through individualized education, health management counselling, and specialized medical care.
The Vascular Improvement Program (VIP) is for people who have had a heart attack, stroke, bypass surgery, angioplasty, artery or heart problems - or if a doctor has indicated that your blood pressure, cholesterol, diabetes, inactivity, weight, smoking, or stress is putting you at higher risk for vascular disease. VIP offers a program of education, exercise sessions, behavioural lifestyle counselling and medical management.

The Vascular Improvement Program is a one-year cardiovascular risk reduction program that provides multidisciplinary Cardiac Rehabilitation services. The primary focus is on post-MI; cardiac surgery and CAD (medical management) in the first year of diagnosis. A Primary prevention program is also available.

A referral from a family doctor or nurse practitioner is required and there is no cost for this program. Virtual and in-person services are currently available to improve access. Consult-only services are not available. All patients are followed over 6 months to a year by a multidisciplinary team including specialist physicians.
